Student Forms
Parent / Student Acknowledgment of Rules Form This form is available in the student profile in the TMS System.
Medical History / Student Annual Physical Form These forms must be completed annually and the current forms are required to be kept on file with the school
Student Transfer Form / Previous Athletic Participation Form (PAPF) These forms must be completed and submitted to TAPPS for approval prior to participation in TAPPS Sanctioned activities
International Student Application for Eligibility All students who are not citizens of the United States must submit the form for approval prior to participation in TAPPS Sanctioned activities
Students not Residing with Parents Application for Eligibility All students who are not residing with their biological or adoptive parents must submit the form for approval prior to participation in TAPPS Sanctioned activities
TAPPS Return to Play Form (Concussion) All students who are diagnosed with a concussion must present this or a similar form prior to returning to practice or competition.
Displaced Student Eligibility Waiver In the event of natural disaster or other such occurrence a waiver process is necessary to establish temporary or long term student eligibility

Appeal to TAPPS Executive Board Formal Appeal to the TAPPS Executive Board (A $300 fee shall be assessed upon receipt of appeal)
Add an Activity Add an Activity for the 2024-25 school year
Drop an Activity Drop an Activity for the 2024-25 school year
TAPPS Good Sportsmanship Reporting Form Please complete this form to recognize a school or its stakeholders for upholding the TAPPS motto; ” Competition with honor”
Application for Waiver of Drone Restriction Please complete this form and receive TAPPS permission prior to any use of a drone or drones in accordance with the TAPPS By-Laws.

Removal from a contest Form Please use this form to report the removal from a contest for coaches, players or fans.  Both schools involved in the contest should report the incident
Request for Activity Deadline Waiver
Request for Medical Waiver This form shall be used to apply for a medical waiver or exception to established TAPPS or activity rules / restrictions.
Request to play Out of State Opponent or Compete Out of State TAPPS shall review and approve all out of state participation by member schools and shall sanction such contests (A $25 fee shall be assessed upon receipt)
